Scheduling an Appointment

To schedule an appointment, please contact our office directly or use our online booking system (if applicable). We aim to offer convenient appointment times that fit your schedule. New patients are encouraged to complete any necessary intake forms prior to their first visit to ensure a smooth experience. If you need to reschedule or cancel an appointment, please notify us at least 24 hours in advance to avoid cancellation fees and to allow us to accommodate other patients in need.

Payment Policies

Payment is due at the time of service unless other arrangements have been made in advance. We accept various forms of payment, including cash, credit/debit cards, and certain insurance plans (subject to verification). Please note that co-pays, deductibles, and any fees not covered by insurance are the patient’s responsibility.

For any additional services such as form completions, prior authorizations, and report preparation, fees may apply. If you have questions regarding billing, our team is available to assist you. We are also in the process of credentialing with additional insurance carriers and will update our accepted plans as this progresses.

Insurance

We are currently in-network with the following insurance providers:

  • Horizon BCBS of NJ
  • Aetna
  • United Healthcare
  • Cigna


For patients with other insurance plans, we are considered out-of-network. In these cases, payment is due at the time of service, but we can provide a superbill upon request, which you may submit to your insurance for potential reimbursement.

Please verify with your insurance carrier to understand your specific coverage, deductibles, and any co-pays or out-of-pocket expenses. Our team is available to assist with any questions regarding insurance and billing.

Cancellation Policy

We request that all appointment cancellations or rescheduling be done at least 24 hours in advance. Failure to cancel within this timeframe may result in a cancellation fee. This policy allows us to accommodate other patients who may need immediate care.

Medical Record

We are committed to protecting your privacy in accordance with HIPAA guidelines. If you need access to your medical records, please submit a formal request, as required by law. All record requests will be processed securely to safeguard your personal health information. A processing fee may apply, depending on the format and quantity of records requested.

Your personal and medical information is safeguarded and only used or shared as necessary for your treatment, billing, or with your explicit consent.
